HOMES

HOMES provides intensive case management services to chronic and vulnerable homeless individuals and families. Chronic Homelessness is defined as a person who has been homeless for one or more years or has had three episodes of homelessness in the past four years. The goal is to provide support, and resources that will enable the homeless to successfully reintegrate into housing.

Clients in HOMES receive a Shelter + Care housing voucher through the Fort Worth Housing Authority. In addition to providing a home to live in, it also enables CCFW to administer intensive case management. In this program, intensive case management includes two client visits each week for the first 90 days in housing, and weekly home visits for the remainder of their service.

HOMES is a strategy of the 10 Year Homeless Plan-Directions Home: Making Homelessness Rare, Short-Term, and Non-Recurring in Fort Worth, TX within 10 Years. All referrals are from the Fort Worth Housing Authority.
